By booking, attending, or participating in Tuning Fork Vibration Therapy sessions with Hay Zen Studio, you acknowledge and agree to the following terms and conditions.

1. Nature of Services

Tuning Fork Vibration Therapy is a complementary and alternative therapy that uses sound frequencies and vibration to support relaxation and overall wellbeing. These services are non-medical in nature and are not intended to diagnose, treat, cure, or prevent any medical or psychological condition.

  • Full name and contact details (phone, email)

  • Emergency contact information

  • Relevant health information (where voluntarily provided)

  • Appointment and session notes

  • Payment and transaction details

2. No Medical Advice

The practitioner does not provide medical, psychological, or psychiatric advice, diagnosis, or treatment. Clients are advised to seek appropriate care from a qualified healthcare professional for any medical or mental health concerns.

3. Client Responsibility

Clients acknowledge that they are responsible for their own health and wellbeing. Participation in sessions is voluntary, and clients accept full responsibility for any outcomes experienced during or after the session.

4. Informed Consent

By attending a session, you confirm that you understand the nature and purpose of Tuning Fork Vibration Therapy. You acknowledge that:

  • Results may vary between individuals

  • No guarantees of outcomes are provided

  • You may withdraw from a session at any time

5. Disclosure of Health Conditions

Clients agree to disclose all relevant health information, including but not limited to:

  • Medical conditions or injuries

  • Pregnancy

  • Mental health conditions

  • Current treatments or medications

Failure to disclose relevant information may affect the safety and effectiveness of the session, and the practitioner accepts no liability in such cases.

6. Contraindications & Suitability

While Tuning Fork Therapy is generally considered safe, certain conditions may require modifications or may not be suitable for treatment. The practitioner reserves the right to decline or modify services where necessary to ensure client safety.

7. Limitation of Liability

To the fullest extent permitted by law, the practitioner shall not be liable for any injury, loss, or damage arising directly or indirectly from the use of services provided, except where required by applicable law. Clients agree to release and hold harmless the practitioner from any claims arising from participation in sessions.

8. Confidentiality & Privacy

All client information is treated as confidential and handled in accordance with applicable privacy laws. Information will not be disclosed without consent, except where required by law or where there is a risk of harm to the client or others.

9. Cancellations & Fees

A minimum of 24 hours’ notice is required for cancellations or rescheduling. Missed appointments or late cancellations may incur a fee at the practitioner’s discretion.

10. Code of Conduct

Clients are expected to behave in a respectful and appropriate manner. The practitioner reserves the right to refuse or terminate a session if a client’s behaviour is deemed inappropriate or unsafe.

11. Governing Law

These terms are governed by the laws of the relevant state or territory in which the services are provided.

12. Agreement

By booking or attending a session, you confirm that you have read, understood, and agree to these Terms of Use & Informed Consent.
